Emergency-Nursing
Emergency nursing is a specialty within the field of professional nursing focusing on the care of patients who require prompt medical attention to avoid long-term disability or death. In addition to addressing "true emergencies," emergency nurses increasingly care for people who are unwilling or unable to get primary medical care elsewhere and come to emergency departments for help. In fact, only a small percentage of emergency department (ED) patients have emergency conditions such as a stroke, heart attack or major trauma. Pediatric Cardiology

Cardiology is a branch of medicine that deals with the disorders of the heart as well as some parts of the circulatory system. The field includes medical diagnosis and treatment of congenital heart defects, coronary artery disease, heart failure, valvular heart disease and electrophysiology. Physicians who specialize in this field of medicine are called cardiologists, a specialty of internal medicine. Pediatric cardiologists are pediatricians who specialize in cardiology. Physicians who specialize in cardiac surgery are called cardiothoracic surgeons or cardiac surgeons, a specialty of general surgery.

Although the cardiovascular system is inextricably linked to blood, cardiology is relatively unconcerned with hematology and its diseases. Some obvious exceptions that affect the function of the heart would be blood tests (electrolyte disturbances, troponins), decreased oxygen carrying capacity (anemia, hypovolemic shock), and coagulopathies.

Cardiovascular Surgery

Cardiovascular surgery treats diseases and conditions of the heart and the blood vessels of the entire body. Common cardiovascular procedures include coronary bypass surgery, aortic or mitral valve replacement or repair, and aneurysm repair. Cardiovascular surgery methods have advanced substantially since 1896, when Dr. Ludwig Rein of Germany performed the first successful cardiac surgery by ligation of a stab wound to the heart accessed through a thoracotomy. Since the 1990s, open heart surgery has been performed using cardiopulmonary bypass that provides oxygen to the body while the heart is steadied and emptied of its blood, allowing for more complicated and protracted surgeries to be performed.

Electrocardiography

Electrocardiography is the process of producing an electrocardiogram (ECG or EKG). It is a graph of voltage versus time of the electrical activity of the heart using electrodes placed on the skin. These electrodes detect the small electrical changes that are a consequence of cardiac muscle depolarization followed by repolarization during each cardiac cycle (heartbeat). Changes in the normal ECG pattern occur in numerous cardiac abnormalities, including cardiac rhythm disturbances (such as atrial fibrillation and ventricular tachycardia), inadequate coronary artery blood flow (such as myocardial ischemia and myocardial infarction), and electrolyte disturbances (such as hypokalemia and hyperkalemia).

Cardiac Pharmacology

Cardiovascular Pharmacology deals with the study of the effects of drugs upon the heart or circulatory system. It mainly contributes to the safety profile of potential new drugs and provides pharmacological data that can be used for optimization of further compounds and the ultimate selection of compounds suitable for clinical development. The cardiac drugs affect the function of the heart in three main ways. They can affect the force of contraction of the heart muscle (inotropic effects), they can affect the frequency of the heart beat or heart rate (chronotropic effects) or they can affect the regularity of the heart beat (rhythmic effects). Drugs can also affect blood vessels by altering the state of compression of the smooth muscle in the vessel wall, altering its diameter and accordingly directing the volume of blood stream. Such medications are classified as vasoconstrictors or vasodilators depending on the smooth muscle lining contraction and relaxation respectively.

Gynecologic/Obstetric Emergencies

Gynecological disorders are a common cause of morbidity among women of reproductive age worldwide. In developing countries, gynecological emergencies present enormous challenges given the weak health infrastructure in these settings. Whereas reports indicate that approximately 1.4 million gynecological emergency visits to emergency department are recorded in the USA accounting for 24.3 visits per 1000 women of reproductive age (15 - 44 years), equivalent data for developing countries are not readily available. However acute gynecological conditions are important causes of morbidity and mortality in these countries and constitute significant public health problems.

This review will highlight common gynecological emergencies in the tropics and discuss current evidence based approaches to their management.

The most common gynecological emergencies are ectopic pregnancy, acute pelvic inflammatory disease, miscarriages and complicated ovarian cysts. Other gynecological conditions which may present as emergencies are menstrual disorders, bleeding gynecological malignancies, coital laceration and sexual assault. In the tropics, ectopic pregnancy and complications of unsafe abortion are the most common life threatening gynecological emergencies.

Endocrine/Neurological Emergencies

Endocrine emergencies are a collection of rare and extreme manifestations of common endocrine disorders that are often triggered by an inciting event, such as an acute infection. An endocrine emergency may be the first presentation of the underlying endocrine disorder. The neurologic and systemic complications of endocrine disorders generally worsen with increasing severity of the endocrine dysfunction. However, the rate of change is important, as rapid alterations may result in significant neurologic dysfunction, while severe but chronic endocrine dysfunction may have only minimal symptoms. As many of these endocrine emergencies can be successfully managed if accurately and promptly diagnosed, clinical neurologists should be aware of the neurologic manifestations of endocrine disorders.

Cardio-toxicity is a condition when there is damage to the heart muscle. It is the occurrence of heart electrophysiology dysfunction or muscle damage.   • What is Cardiovascular Disease?
  • Cardiovascular Diseases (CVD) are a group of disorders affecting the heart and blood vessels and are the most common cause of death globally. According to WHO1, CVD contributed to approximately 31% or 17.9 million deaths globally in the year 2016. According to Our analyst, some of the major diseases that contribute to CVD based on sales are:
  • Hypertension due to high blood pressure
  • Atrial fibrillation caused by abnormal heart rhythm, also known as an arrhythmia
  • Chronic ischemic heart disease caused by reduced blood supply to the heart
  • Stroke caused due to an interruption of the blood supply to part of the brain
  • Heart failure caused by inability of the heart to pump blood around the body efficiently
  • Angina caused when a part of the heart does not receive enough oxygen
  • Myocardial infarction due to interrupted blood flow damaging or destroying part of the heart muscle
  • Although the causes of the many different types of CVD are not clear-cut, the risk of getting these heart diseases increases when an individual is suffering from high blood pressure, high cholesterol, or diabetes or is obese or has a family history of CVD.   • Therapy Landscape of Major Heart Diseases
  • There is a wide range of medications available for heart disease, a number of which are taken for chronic use. Examples include:
  • Statins - cholesterol-lowering medications used for lowering LDL cholesterol levels
  • Anticoagulants - for preventing blood clots. Often prescribed to prevent first or recurrent stroke
  • Diuretics - to maintain blood pressure
  • Beta-Blockers – for lowering blood pressure, heart attack and heart failure
  • Angiotensin - Converting Enzyme (ACE) inhibitors - for heart failure and high blood pressure
  • Calcium channel blockers - for reducing the workload of the heart.

  • According to Forecast Link, the market for major heart disease prescription drug sales contributed $67 billion in 2018 and has only grown at a CAGR of 0.6% since 2014, mainly due to the significant generic erosion of a blockbuster hypertension brand from late 2013. However, going forward, this market is expected to grow at a CAGR of 5.5% (2018-24) post which impending loss of protection (LoP) and generic entry will decrease the market growth at a CAGR of -2.2% (2024-28).
  • The hypertension market is the biggest among the other heart diseases with prescription drug sales contributing $32 billion in 2018, and is expected to grow at a CAGR of 1.7% to 2028. This is followed by atrial fibrillation which had the second-highest sales of $18 billion in 2018 and will maintain its position out to 2028, growing at a CAGR of 1%. The heart disease market forecast for 2028 is shown in Figure 1.

  • The US is the biggest market for heart disease and will continue to dominate in 2028 totaling $26.7 billion sales owing to their higher cost of treatments. In terms of percentage contribution in sales it accounts for 32% in 2028 and is expected to grow at a CAGR of 2.4% from 2018 to 2028.
  • Europe and APAC will also see healthy growth in terms of heart disease market while the market in Japan, MENA and AUNZ will be stable. Europe and APAC are expected to account for 27% and 22% of the global market respectively, and the remaining regions will contribute 20% in 2028.
  • In terms of sales growth percentages, maximum growth in sales will be seen in APAC countries, driven by new launches and an increase in number of patient days. While on the volume front, MENA will observe the highest growth followed by Americas. Drug treated patient-days volume is forecast to show slight growth, with an average global growth of 1.4% over the next 10 years. Sales and volume forecast growth rates from 2018 to 2028 are shown in Figure 2.
  • Growth due to new launches overshadowed by the loss of protection events
  • According to Forecast Link, the heart disease market will see as many as 25 launch events take place by 2023. The impact of these new launches will expand the market by approximately $5.8 billion in 2028 (Figure 3). However, due to the impending LoP of some key drugs and the consequent entry of low-priced generics, there will be a net reduction in the global market of over $21.4 billion in sales, by 2028. IQVIA’s Forecast Link predicts that at least 137 LoP events will take place between 2019-2028.

  • Conclusion
  • As of 2018, 32% of the drugs in being taken by patients for CVD were generics while branded products contributed to 64%. However, there will be a shift in this trend over the next 10 years as the proportion of generic use increases, with as much as 44% being generic by 2028. Even with the launch of 25 new molecules in this disease area, the loss of protection of 137 brands will see the usage of generics among patients greatly increase? Unless new therapies are introduced into the market, the dominance of generics will pertain, and some geographies may observe declining sales in this disease area.
